Products / Medical Forms for New and Returning Athletes
All Special Olympics Kansas athletes are required to have an up-to-date medical release form in order to participate in sporting events. Medical release forms expire on December 31 of even years (e.g. 2006, 2008), and therefore must be updated every two years.
New Athletes: New athletes must have a physician or nurse practitioner perform a routine physical and complete the Medical Release Form below.
Returning Athletes: If a returning athlete has one of the conditions listed below, they must have a physician or nurse practitioner perform a routine physical and complete a Medical Release Form every two years.
If a returning athlete does not have one of the below conditions, a parent or legal guardian should only complete the Health Info Update Form every two years.
Heart disease / heart defect / high blood pressure Chest pain Seizures / epilepsy/fainting spells Diabetes Concussion or serious head injury Major surgery or serious illness Blindness / visual problem Asthma *Down Syndrome
Down Syndrome: Athletes with Down Syndrome must complete the Medical Addendum for Down Syndrome every two years, along with the Medical Release Form. If an athlete with Down Syndrome has Atlantoaxial Subluxation, a parent or legal guardian must also complete the Agreement Addendum for Down Syndrome.

All completed forms should be returned to the Douglas County Special Olympics coach listed below via mail, email, or in person.
