Nominations are being accepted for the 12th annual Phoenix Awards, a project of the Lawrence Arts Commission. Eight awards will be presented, recognizing area residents for outstanding achievement in the arts, during a ceremony at 2 p.m. Nov. 4 at the Lawrence Arts Center, 940 N.H.

A reception will follow. Both the awards ceremony and the reception are open to the public.

Anyone may nominate a person for an award in one of 10 categories: arts advocate, arts administrator, design arts, educator in the arts, literary arts, musical arts, performing arts, visual arts, volunteer in the arts and exceptional artistic achievement.

Nomination forms, which include lists of past winners and award designers, are available at the Lawrence Arts Center; City Hall, 6 E. Sixth St.; and the Lawrence Public Library, 707 Vt. The form also may be downloaded at www.lawrenceartscommission.org.

Supporting materials for the nominee may include letters, media citations, lists of accomplishments and other documents.
