The local health advocacy coalition LiveWell Douglas County is seeking feedback from area residents as it continues to shape its Community Health Plan.

The group has posted a brief survey online at surveymonkey.com/r/DK2RSKV seeking input. It asks participants to rank the importance of various actions to address physical activity and the built environment, as well as access to healthy food and reducing hunger.
