How to check whether you have correctly filled out the county’s vaccine interest form

Douglas County residents can now check to see whether they have correctly filled out the county’s vaccine interest form.

The form allows residents to indicate their level of interest in receiving a COVID-19 vaccine and helps the Unified Command Vaccination Unit determine which phase people might qualify for. The form helps providers notify individuals when the vaccine becomes available and gives them instructions on how to schedule an appointment.

To find out if you filled out the form correctly, search for your record online at dgcoks.org/vaccineinterestformsearch. The list of names will be updated at 11 a.m. weekdays. If no record is found, fill out the form again online at dgcoks.org/vaccineinterestform.
