Helicopter crash likely caused by pilot error

? A 2006 helicopter crash that killed a cameraman and hurt two others was likely caused by pilot error, according to the National Transportation Safety Board.

The crash occurred June 30, 2006, during filming of “The Final Season,” a movie about an Iowa high school baseball team that won the state baseball championship. The movie premieres Sunday in Cedar Rapids.

Cameraman Roland Schlotzhauer, of Lenexa, Kan., died in the crash. Pilot Rick Green and film producer Tony Wilson were seriously injured.

A wrongful death lawsuit filed by the cameraman’s widow, Kathryn Schlotzhauer, against Green, Wilson and 17 others is pending in Polk County District Court.

A ruling last week by NTSB investigators found that the accident’s probable cause was “the pilot’s failure to maintain clearance from power lines.”

The helicopter was traveling between 30 feet and 50 feet above ground at about 55 mph, filming a caravan of cars traveling along Highway 151. The crash investigation included viewing the film, which ended abruptly as the copter was about 200 feet south of the highway.

“There was no evidence of any avoidance or maneuver prior to the end of the recording,” the report said.

Investigators didn’t find any mechanical problems.

The report also found the pilot didn’t have a waiver required to fly a helicopter during filming.
