End-of-life care groups can apply for award

Community-based coalitions working to improve care for the dying may apply for one of five Community Coalition Awards of Excellence from Rallying Points. Rallying Points is an initiative of the Robert Wood Johnson Foundation, which helps local coalitions improve end-of-life care.

Each award will grant $15,000 to the winner. Recipients will be judged on identifying community needs for better care of the dying, promoting health system changes to care for the dying, developing and implementing specific end-of-life projects and providing guidance to new coalitions through mentoring.

Applicants must be involved with Rallying Points and fill out a questionnaire on the group’s Web site, www.rallyingpoints. org.
