Kansas City, Mo. ? Safety procedures were not followed properly at a housing construction site where a trench collapsed, killing a worker, an official with the Occupational Safety and Health Administration said Thursday.

John W. Anderson, 51, of Excelsior Springs, was killed Wednesday at the northern Kansas City site when a trench in which he was working caved in. His body was recovered about 10 p.m., after a rescue attempt that started shortly before 1 p.m.

The trench was 10 to 15 feet deep.

Barb Theriot, OSHA’s director in the Kansas City office, said it did not appear that the site had wood and steel shoring, sloped edges or a steel or concrete safety box, as required by federal safety regulations when trenches are more than 5 feet deep.
