Tools for Better Health: Living with Chronic Conditions

Developed by researchers at Stanford University Patient Education Research Center, this evidence based, highly interactive six week workshop series is being offered free of charge.

It is appropriate for individuals managing any type of chronic health condition

Chronic Condition Self-Management Program

Subjects covered include:

  1. Goal setting
  2. Exercise
  3. Communication
  4. Nutrition
  5. Managing emotions and stress
  6. Problem solving
  7. Symptom management

Participants will make weekly action plans, share experiences, and help each other solve problems they encounter in creating and carrying out their self-management program. Materials presented in the workshop have been reviewed by physicians, diabetes educators, dietitians, and other health professionals both at Stanford and in the community.
